Optional permission for adult subjects
Lets an identifiable adult choose whether selected photographs may appear on business social media, the website, in a portfolio, or in other marketing.
Declining promotional use does not affect photography services.
Required when a minor participates
Records a parent or legal guardian's authority, approval for the minor to participate, and a separate choice about promotional use.
A parent, legal guardian, or person with documented legal authority must complete it.
Property, business, rental, or private location
Documents permission to enter and photograph a controlled location, the intended use, access limits, privacy concerns, and the signer's authority.
This is the standard business practice for commissioned property and private-location sessions.
