Service Oklahoma Form 718 - revised 09/2025
Use only when: the current Oklahoma title on record is electronic. It is not a universal bill of sale for every vehicle or personal-property transaction.
Service Oklahoma Form 701-6 - revised 07/2025
Use for: an Oklahoma certificate of title for a vehicle, trailer, or manufactured home when the applicable title process requires Form 701-6.
Service Oklahoma Form 701-7 - revised 07/2025
Use for: requesting a replacement title for a vehicle, boat, or motor when the record owner's title has been misplaced or destroyed.
Service Oklahoma Form 729 - revised 01/2023
Use when: an applicable vehicle ownership transfer requires a separate federal odometer disclosure. Both transferor and transferee information is requested.
Service Oklahoma Form 722-1 - revised 01/2023
Use only when: no contract or bill of sale was prepared and the purchaser cannot obtain one from the seller. This declaration establishes purchase price; it does not transfer ownership.
Service Oklahoma Form 773 - revised 01/2023
Use for: optionally notifying Service Oklahoma of an ownership assignment. Filing this notice does not itself transfer title, and the form states a filing fee.
Form 718 applies only to an electronic Oklahoma title. Form 773 is only a notice and does not transfer title. Service Oklahoma may require additional documents based on the transaction.
Section 2 - Direct-source forms

Residency
There is no single universal Oklahoma affidavit of residency for every purpose. Schools, licensing programs, courts, landlords, and agencies set their own wording and acceptable proof. Obtain the exact form from the organization requesting it.
Vehicle estate transfer
Service Oklahoma limits this form to a vehicle subject to disposition by will when the stated estate-value and supporting-document conditions are met.
Family vehicle transfer
For qualifying family vehicle transfers. Confirm the eligible relationship, tax treatment, title documents, and current requirements with Service Oklahoma.
Deceased owner
For a qualifying vehicle, boat, or outboard-motor transfer when no executor or administrator has been appointed. Relationship and death-certificate requirements apply.
Lost assigned title
Only for an applicant who received a properly assigned Oklahoma title and then lost it. Confirming ownership-transfer documentation is required.
Ownership documentation problem
Contains sworn ownership, indemnity, and documentation statements. Use only when Service Oklahoma confirms this affidavit fits the ownership problem.
Legal authority
A power of attorney can grant significant legal authority. Use legal-aid information or consult an Oklahoma attorney instead of selecting a generic form through the notary.

Section 3 - Notarial certificate downloads
These one-page certificates reproduce Oklahoma's statutory short-form wording. They are not stand-alone legal documents. The document recipient or another authorized party must identify the required notarial act.
Acknowledgment
For an individual acknowledging execution of an identified instrument.
Acknowledgment
For a signer acknowledging an instrument in a stated representative role.
Oath or affirmation
Often called a jurat; the signer swears or affirms the truthfulness of the statement.
Signature witnessed
For a signature made or acknowledged in the notarial officer's presence.
Copy certification
For permitted copy attestation; some records require certification by the official custodian.
The notarial officer completes it only as part of the requested notarial act and connects it to the document it describes.
